Job Title: Rehabilitation Case Manager

Job Detail: We are currently looking to recruit on behalf of an international rehabilitation provider a Rehabilitation Case Manager for their Manchester office.
Duties/ Responsibilities
The sucessful candidate will be responsible for co-ordinating quality healthcare services to meet an individual\'s specific healthcare needs in a cost effective manner to promote positive outcomes. You will ensure service standards for case management are adhered to and in line with company procedures and business strategies and achieve their monthly income targets.
Main duties will include working independently and across professional boundaries, managing a wide range of cases, management of an active case load of between 100 and 130 cases. Liaison with service providers and internal and external clients to negotiate care packages and treatment programmes. Maintain regular and effective contact with the insurer, external clients, and other key parties involved in the rehabilitation process by telephone and in writing. Meet performance targets as specified. Conduct client visits to manage individual accounts effectively and to develop strong working relationships.
Requirements
The successful candidate will be a registered medical professional for example RGN, Physiotherapist, OT etc. with post registration medical / clinical experience in either the public or private sector.
You will have excellent communication skills (both written and verbal)and you will have the ability to work autonomously for the majority of the time incorporating independent decision making, problem solving and creative thinking skills. You will also have proficient IT skills in Word, Excel, Powerpoint and MS Outlook. Occupational Health experience preferable.
Flexibility is required, as there is UK travel/overnight stays, this generally would not exceed three times per month for travelling and one to two overnight stays on a quarterly basis.
Salary for this position will be between £29,000 and £31,000 for 34 hours per week.
